- New Pharmacy Kit: Going forward, only the infusion sets listed below will be available through your pharmacy benefits. These new infusion sets will be packaged together with T:Slim cartridges in a single, convenient Pharmacy Kit.
- Cost Consideration: Each Pharmacy Kit will count as one co-pay. One Kit = One Co-Pay

1. Why is my current infusion set being discontinued?
Tandem is consolidating its product lineup to the most commonly used infusion sets for Pharmacy-benefit coverage. Your current infusion set may still be available under your Medical benefits. Going forward, for Pharmacy benefits, only the options listed on the form will be available.
2. Do I have to switch, or can I keep using my current set?
Yes, you must switch if you want to continue using your T:Slim pump. Other infusion sets will no longer be available.
3. What is included in the new Pharmacy Kit?
Each kit includes 10 infusion sets and 10 cartridges, packaged together for convenience.
4. How much will the new kits cost me?
Most patients will see cost savings, since each kit counts as one copay instead of two.
5. Will my copay or coverage change?
For most patients, coverage remains the same. If any issues arise, Express Rx will work with you and your insurance plan.
6. Can I choose which Pharmacy Kit I want?
Yes. You can choose from the three approved options.
7. How do I let Express Rx know my choice?

8. Do I need a new prescription from my doctor?
Yes, but you do not need to worry — once you make your selection, we will contact your doctor for you.

We will not be able to request a new kit from your doctor, and you may run out of supplies.
